* Information on the processing of personal data in connection with the organization of the conference
Pursuant to Article 13 in connection with Article 26 of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation), hereinafter referred to as the “Regulation” or “RODO”, we inform you that:

1. the Joint Administrators of your personal data are the Conference Co-Organizers:
(a) Monika Wachowska, conducting business under the name CONGRESSUS with its registered office in Toruń, address: Nad Strugą 2/9 Street 87-100 Toruń, NIP 879-195-31-13 REGON 523603819 hereinafter “Co-administrator 1”,
b) Indywidualna Praktyka Lekarska Błażej Pruszczyński with its registered office in Łódź, address: Łagiewnicka 169, 91-863 Łódź, NIP: 7262278562 hereinafter “Co-Administrator 2”,
c) Centrum Medyczne Querqus located at 20 Tymienieckiego Street, 90-349 Łódź, run by Pruszcz Holding sp. z o.o. sp. k. with its registered office in Warsaw at Plac Bankowy 2, 00-095 Warsaw, KRS No.: 0000718162 REGON: 369489400, NIP: 5252739664, hereinafter referred to as “Co-Administrator 3”,

3. your personal data are processed for the following purposes and legal bases:
(a) Article 6(1)(f) and Article 6(1)(c) of the RODO, for the preparation, execution and settlement of the Conference, including registration of participation in the Conference, contact for the transmission of organizational information, transmission of materials related to the Conference, posting of information regarding invited guests,
(b) Article 6(1)(a) of the RODO, with respect to consenting to the processing of personal data in the form of an image in connection with the taking and publication of photos of guests and participants as part of materials promoting the Conference, the Conference website, and the publication of photos taken during the Conference in social media and post-Conference materials,
(c) Article 6(1)(f) in order to assert or defend against claims related to the organization of the conference,

(4) The Joint Administrators will process the following personal data of participants and invited guests: name, image, phone number, email address.

(5) Co-administrators will collect personal data obtained directly from participants and invited guests.

(6) Your provision of personal data is voluntary, but necessary to conduct and organize the conference.

7. as a rule, your data will be stored for a period of up to 90 days, unless the basis for their processing is determined by applicable law or justified by the ongoing proceedings. Data collected on the basis of consent, as a rule, will be stored until you withdraw your consent or until they are used for the purpose for which they were given (achievement of the purpose of processing).

(8) Recipients of your personal data may be entities providing services to the Joint Administrators and subcontractors (processors), for the purpose and to the extent necessary to carry out the instructions of the Joint Administrators on the basis of contracts, as well as authorities and entities specified by law.

(9) The Joint Administrators do not transfer personal data to countries outside the European Economic Area.

(10) You have the right to: request access to your data, the right to rectify your data, and, to the extent provided by applicable law: the right to restrict processing, the right to request erasure of your data, the right to object to processing, the right to data portability.

(11) Personal data will not be subject to automated decision-making (including profiling).

(12) You also have the right to lodge a complaint to the supervisory authority, i.e. the President of the Office for Personal Data Protection (ul. Stawki 2, 00-193 Warsaw).
13. As part of the concluded co-management agreement, the Joint Administrators have agreed on the following terms of cooperation:
(a) each of the Joint Administrators shall, on its own, ensure the security of the processing of personal data by implementing appropriate technical and organizational measures, adequate to the type of personal data and the risk of violation of the rights of data subjects,
b) the information obligation under Article 13 of the RODO with respect to persons whose personal data are processed shall be fulfilled by Joint Controller No. 1,
(c) each of the Joint Administrators shall, on its own, maintain the necessary documentation of personal data protection,
(d) Subject to Section 8, only employees or associates of the Co-Administrator who have been familiarized with the regulations governing the protection of personal data and who have received authorization to process personal data issued by the Co-Administrator may have access to personal data,
(e) The Joint Controllers have allowed the possibility of using the support of subcontractors who are data processors, the Joint Controller using the services of a processor shall be responsible for the actions of the processor as for its own. Each Joint Controller may raise a documented objection to entrusting the processing of personal data to a processor for legitimate reasons.
(f) Each Joint Controller shall be liable for damages caused by its actions for failure to comply with the obligations that the RODO imposes directly on the Controller,
